Please either download or print the waiver and registration form, fill it out completely, and email it back to us at firstname.lastname@example.org
. These forms must be filled out in their entirety before your player will be allowed to participate. We would prefer both were emailed in to us at least two (2) weeks prior to the start of the selected session but at the very least, the REGISTRATION form must be completed and emailed to us at least two (2) weeks prior to the start of your selected session to ensure that we get the correct shirts and hats needed for each participant.
